May 8, 1930 - January 13, 2023 Phyllis Edythe Applegate, born May 08, 1930, in Providence Rhode Island, the fourth of five children of parents William and Edythe Costello Applegate, passed away on January 13, 2023, in Los Angeles CA.

As an active four-decade member of SAG/AFTRA, Phyllis enjoyed a successful career with many memorable feature roles in: The Five Heartbeats, Big Momma's House, Jagged Edge, Beauty Shop and Go Tell Sol.

A private memorial will be held.
